SECTION 1 · BUSINESS CONTEXT

Tell Us About Your Business

Basic context that frames everything else. Takes about 3 minutes.
As it appears on your EIN, bank account, or Secretary of State registration.
CEO, COO, Operations Lead, Founder, etc.
One or two words — e.g., Professional Services, SaaS, Food Service, Nonprofit.

Be honest — this is where ClearPath focuses its work. Bottlenecks, manual workarounds, "this doesn't scale" feelings, or things your team complains about.

Where Is It Hurting?

Rate the severity of pain across common friction categories, then tell us the story. About 4 minutes.
Why we ask this: This is the raw input for your Friction & Waste Register. The items you rate most severe will drive our scoping conversation and shape which functions get mapped first.

Why we ask this: This feeds the AI Readiness Report and the Current State Systems Map. Be honest about what you actually use — if you paid for a CRM but no one logs in, that's a spreadsheet, not a CRM.
List the systems that are actively in use today. If a function isn't on a system, leave it blank or write "spreadsheets" or "none."
